Will travel throughout this area!   The Licensed Practical Nurse (LPN) provides skilled nursing services to patients in their homes in accordance with the patient’s established plan of care, physician/provider orders, Pennsylvania nursing laws and regulations, applicable federal requirements, and agency policies and procedures. The LPN practices within the scope of practical nursing in Pennsylvania and performs nursing services under the direction of a Registered Nurse (RN), physician, or other authorized prescriber, as applicable. The LPN works collaboratively with the RN, physician/provider, patient, family/caregivers, and interdisciplinary team to promote patient safety, continuity of care, functional independence, and appropriate self-care. Essential Duties and Responsibilities Patient Care · Provide skilled nursing care in the patient's home according to the established plan of care, physician/provider orders, agency policies, and applicable laws and regulations. · Perform nursing interventions and treatments that are within the LPN's education, training, demonstrated competency, and Pennsylvania scope of practice. · Observe the patient's physical, functional, behavioral, and emotional status and promptly communicate significant changes to the supervising RN and/or physician/provider according to agency policy. · Administer medications and treatments as ordered and within the LPN's legal scope of practice, agency policy, competency, and applicable requirements. · Monitor and document the patient's response to medications, treatments, and nursing interventions. · Promote patient safety, dignity, privacy, independence, and comfort in the home environment. · Document nursing interventions, treatments, medications administered, patient response, education/reinforcement, and significant observations in accordance with agency requirements. · Maintain patient records in accordance with agency policies, Pennsylvania requirements, applicable federal requirements, and accepted standards of nursing practice. Care Coordination and Communication · Participate in interdisciplinary communication and care coordination as directed. · Communicate effectively with patients, families, caregivers, physicians/providers, RNs, therapists, home health aides, and other members of the care team. Qualifications Required · Current, unrestricted Licensed Practical Nurse (LPN) license in the Commonwealth of Pennsylvania. · Graduation from an approved practical nursing program. · Current CPR/BLS certification, if required by the agency or applicable payer. · 2 years previous home-health, community-health, or rehabilitation nursing experience. · Must have a driver’s license and reliable transportation, and the ability to travel to patients' homes and perform the physical requirements associated with home-health nursing. Physical and Environmental Requirements · Ability to travel to and provide care in patients' private residences and other authorized home settings. · Ability to stand, walk, bend, kneel, reach, and perform routine nursing procedures. · Ability to safely assist with positioning, transfers, and mobility within the limits of the employee's training and agency policies. · Ability to work in environments with varying space, temperature, cleanliness, and accessibility conditions.
