Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Coordinate with nursing partner to ensure complete coverage of care. Appropriately evaluates the patient, initially and periodically throughout care. Administer medications, injections, IV infusions. Perform wound care, catheter care and ostomy care following strict protocols. Coordinate care plans with physicians and other healthcare professionals while maintaining accurate medical documentation within EMR/EHR systems like Epic, Cerner, or Athenahealth. Provide education on disease management, medication administration, nutrition, and self-care techniques to patients and their families. Monitor patients’ progress through diagnostic evaluations and physiological assessments to adjust care plans accordingly. Ensure compliance with HIPAA regulations and infection control standards during all patient interactions. Requirements Licensed in Colorado as a Registered Nurse or Licensed Practical Nurse without stipulations.
