Description At Hospice of the Piedmont, our Social Workers are a vital source of comfort, guidance, and support for patients and families facing life's most difficult moments. As part of our interdisciplinary team, you'll help individuals navigate complex emotional, social, and practical challenges while ensuring each patient's wishes, dignity, and quality of life remain at the center of care. What You'll Do As A Hospice Social Worker Support patients and families through the emotional and practical challenges of serious illness. Provide counseling, care planning, and resource coordination. Help families navigate advance directives, care transitions, and end-of-life decisions. Facilitate grief and bereavement support services. Collaborate with an interdisciplinary hospice team to deliver compassionate, patient-centered care. Document services and maintain compliance with hospice standards. Participate in on-call coverage as scheduled. What It Takes to Succeed Compassionate and empathetic approach to patient and family care  Strong counseling, communication, and active listening skills  Ability to navigate complex emotional, social, and family dynamics  Excellent critical thinking, problem-solving, and resource coordination skills  Collaborative team player who thrives in an interdisciplinary environment  Strong organizational skills with attention to detail and documentation accuracy  Commitment to honoring patient dignity, choice, and quality of life Qualifications Master's degree in Social Work (MSW) preferred; Bachelor's degree in Social Work, Psychology, Sociology, or related field required.  LCSW licensure preferred.  Minimum of two years of social work experience, including at least one year in a healthcare setting.  Clinical experience working with individuals, families, and groups.
