Position Summary: The referral coordinator position is responsible to ensure patients have access to specialty care services by developing exceptional relationships with HealthPoint providers, patients and community partners to ensure specialty care needs are met. As a referral coordinator you will be responsible for tracking referral statuses, coordinating specialty care for patients and working with all parties to ensure an exceptional patient experience. Compensation is dependent on skills and experience. Your contribution to the team includes: Responsible for verifying eligibility, dates of coverage and benefits for specialty services with a high level of detail to ensure minimal disruptions or delays in care for patients Process referrals by coordinating chart notes, recent labs, diagnostics and other required documents to adhere to HealthPoint and Insurance guidelines related to referral requirements Communicate, problem solve and prioritize issues with patients, external specialists, HealthPoint providers, HealthPoint Managed Care department and health plans as appropriate. Adhere to HealthPoint policy and HIPAA laws to ensure patients information is protected and documented safely Act as liaison between Managed Care insurances and HealthPoint providers to coordinate prior authorizations for patient specialty care and diagnostics Develop and maintain contacts with specialists and diagnostic facilities and actively maintain master specialist list and accepted insurances Manage incoming messages and calls to ensure continuity of care for patients’ access specialty providers Tracking of high-risk referrals month to month and maintain consistent communication with external partners to ensure our patients receive care within the appropriate time Regular review of monthly reporting to ensure adherence to monthly processing times, and all referral statuses are up to date Update referral database with consultative reports received and print out additional reports as needed. Communicate approval or denial of referral authorization to patient and referring provider. Maintains good attendance, is punctual and works full scheduled shift is a condition of employment. Demonstrates respectful, professional and appropriate behavior that supports a team oriented work environment. Identify patients that may be eligible for case management services outside HealthPoint. Perform other duties as assigned by supervisor. Must have’s you’ll need to be successful: Associate’s degree (AA) or equivalent from a two-year college or technical school and two years’ medical experience in a medical setting; or equivalent combination of education and experience. Ability to read and interpret technical and other complex documents. Ability to write routine correspondence such as letters and memos. Ability to present information in one-on-one and small group situations to internal and/or external patients. Understanding of medical terminology preferred. Ability to define problems, upload data, problem solve, establish facts and draw valid conclusions. Ability to interpret an extensive variety of technical instruction. Constructive thinking and alternative short-to long-term solutions must be considered. Action plans must be developed and implemented. Possesses basic operating knowledge of computers. Beginning level of Word, Excel and Outlook required. Ability to type 35wpm.
