The NewPath Child & Family Solutions Case Manager is responsible for assisting clients and families in achieving stated goals and objectives and maximizing independence and daily functioning through interventions and advocacy in our Outpatient Program.   Location: This position is located at our NewPath Central Campus, 4721 Reading Rd. Coordinate the necessary evaluations and assessments in order to identify each client's needs, DLA-20, ISP, PHQ9, Ohio Scales etc. Provide transportation as appropriate and necessary to reach identified goals. Maintain and model appropriate interpersonal relationships. Deliver culturally informed services. Maintain professional objectivity and self-discipline. Adhere to agency risk assessments, policies, protocols, safety questionnaire, on-call procedure, safety planning, and corporate compliance. Ensure that required elements of the ECR/EHR are present, current and in accordance with agency certification and documentation standards.   Qualifications: Minimum of High School diploma and 5 years' experience with SED children; OR a Bachelor's degree in relevant field and at least 6 months experience working with SED children. Excellent written and verbal skills. Ability to work flexible hours, including some evenings and weekends. Initiative and ability to work independently; A willingness and comfort level in providing community-based services to a diverse population of clients; Excellent problem-solving skills and the ability to remain calm and professional in crisis situation. Must be a minimum of 21 years of age, have a valid driver's license and proof of current auto insurance with no more than 5 points on your MVR.
