Description   AMHC is seeking a compassionate, motivated, and organized Full-Time Adult Community Case Manager in Machias to empower adults living with long-term mental health challenges to achieve greater independence, wellness, and success in their communities. Position Summary The Adult Community Case Manager provides community-based case management services to adults with long-term psychiatric disabilities. Working as part of a multidisciplinary team, this position helps clients identify goals, access resources, develop independent living skills, and improve overall wellness using SAMHSA's Eight Dimensions of Wellness and the psychiatric rehabilitation model. What You'll Do: Build Meaningful Relationships Develop therapeutic relationships with clients using a recovery-oriented, person-centered approach.  Engage individuals who may be difficult to reach and promote hope, independence, and self-advocacy.  Support clients in achieving their personal wellness goals.  Care Coordination & Case Management Coordinate individualized services based on each client's treatment or support plan.  Connect clients with medical, behavioral health, housing, employment, financial, educational, and community resources.  Collaborate with healthcare providers, community partners, and multidisciplinary treatment teams.  Wellness & Skill Development Assist clients in building independent living and coping skills.  Facilitate or lead psychoeducational and wellness groups.  Provide supportive counseling and education to clients and, when appropriate, their families or natural supports.  Crisis Support Provide crisis intervention and ongoing support as needed.  Help clients navigate challenges while promoting stability and community integration.  Documentation & Compliance Maintain accurate clinical documentation and case records.  Meet productivity standards and agency documentation requirements.  Participate in team meetings, professional development, and quality improvement initiatives.  What We're Looking For We're seeking someone who is: Compassionate and committed to recovery-oriented care  Organized with strong communication and problem-solving skills  Comfortable working independently and as part of a collaborative team  Passionate about helping individuals achieve greater independence and wellness  Flexible and adaptable in a community-based work environment  Work Environment This position provides services in a variety of community settings, including client homes, healthcare facilities, and community locations throughout the service area. Travel is required. Physical requirements include: Ability to sit, stand, walk, and use standard office equipment  Ability to occasionally lift up to 25 pounds  Reasonable accommodations available for qualified individuals with disabilities  Schedule Full-time, non-exempt position  Primarily Monday–Friday, 8:00 a.m. – 5:00 p.m.  Occasional evening or extended hours may be required to meet client needs  Supervision Reports to the Community Case Management Supervisor.
